To better assist our patients and families with their required medical forms, we're asking families to:
Mail, fax, or drop off your school, daycare, and/or camp forms to our clinic
Before submitting to our clinic, please ensure you've reviewed and thoroughly filled out your form(s) with as much information as possible, including your child's updated height and weight. This will help ensure our staff is able to swiftly complete the healthcare provider's information on your form(s)
Patients/parent(s) may request their Asthma Action Plan (AAP) or Emergency Action Plan (EAP) by calling our clinic at 952-546-6866
Please allow 2 weeks for forms to be completed
We are not able to fill out forms for patients who have not been seen within the last year
Forms will either be returned to the family’s home address on file via mail OR the family may pick up forms in the clinic. We are not able to fax or email the form(s)
We do not fax or email form(s) directly to your institution/organization. Each family/patient is responsible for submitting forms to their specific school, daycare, and/or camp

We Come to You with Telehealth!
While scheduled in-clinic appointments and services are always welcomed and available, Dr. Wexler and our provider team have expanded access to care through telehealth. Telehealth is available to both new and existing patients, and is a convenient way to receive high quality care from our team from the comfort of your home. Our telehealth appointments allow for a face-to-face visit with one of our providers and can be scheduled during regular clinic hours. Additionally, these virtual visits can take place on your computer or phone. Instructions to access your telehealth appointment are given when scheduling. Telehealth visits typically cost the same as an office visit and are covered by most insurance plans.
